Hi Kate
I have a 680 in GMAT. Q49/V34. I have 16 + years of work experience in Tier 1 IT firms. I would want to do PhD on Business Analytics or Data Science only. Should I apply to top 5 .e.g. stanford, HBS etc

Hate to burst your bubble but that GMAT score is terrible for top 5 PhD programs, particularly in Biz Analytics or Data Science. Best case, maybe a top 20 school.
Your quant score is okay, but the verbal is seriously dragging you down. Unless there’s something spectacular – in a department’s opinion, not yours – your admission chances are very, very low.
If you’re not moving forward in your career, a PhD is not likely to fix that. If you just want a change, there are more financially rewarding ways to get a change. Choose:

  1. Work on verbal to pull up GMAT (aim for 730+), or
  2. find a new path that doesn’t involve the PhD.

Make it happen!
